Hammer is taught by Agrashab the troll in Sun Vale when you become Friends with him. Hammer Level 51-60 is taught by Falkrin Overstrike in Amulna.
Equipment
Lucky Belt of the Woodland Warrior increases damage dealt with Hammer and Druid combat skills.
Lucky Belt of the Barbarian increases damage dealt with Hammer and Shield combat skills.
Lucky Belt of the Mindfist increases damage dealt with Hammer and Mentalism combat skills.
